When creating your Lead Gen Forms, you’re required to include a privacy policy URL. You should also describe how you’ll use the collected leads. To obtain the members’ specific consent for each of the uses you describe, you must manually enable one or more disclosure checkboxes during form creation for them to appear on your forms next to the respective use descriptions.
Privacy policy fields
When you create a Lead Gen Form you’re required to enter a privacy policy URL in the Data privacy section. You can also add additional privacy policy text that will appear on the form below the form questions or add custom checkboxes to outline the specific types of consent you need to collect.
- Privacy policy URL (required) - This web page should be solely dedicated to your privacy policy.
- Privacy policy text (optional) - Appears on the form below the link to your privacy policy. It can include:
- Your primary use description stating what you intend to do with the member's information they're choosing to submit.
- Highlighting relevant provisions from your privacy policy.
If you need to collect more specific consent, you can add custom checkboxes in the Lead details & custom questions section. Examples of custom checkbox questions may include:
- We’ll use your information to process your request for information.
- We’ll use your information to register you to receive our newsletters.
- We’ll use your information to contact you about our services and products.
- We’ll use your information to contact you about our events.
Disclosure checkboxes
To get the members’ specific consent for any additional ways you intend to use their information, you have the option to use our disclosure checkboxes. Disclosure checkboxes are optional when creating a Lead Gen Form.
To create a disclosure checkbox:

In the Data privacy section of your Lead Gen Form, click
Add checkbox.- One form can display up to five checkboxes.
- One form can display up to five checkboxes.

Enter the terms and conditions in the description that you’d like your campaign audience to see next to your checkbox in the Checkbox message field.
- Maximum of 500 characters.

You can add hyperlinks in your checkbox message description by clicking the link icon and entering the URL.

If you’d like your checkbox to be a requirement for form submission, select the Required box.
